Most of what you describe is consistent with not allowing enough time for the strobe to recycle. If your setup wasn't asking for a full dump each time, you would eventually empty the strobe capacitor. The easiest way to test this is to watch the strobe's ready light to make sure it comes on before you fire. If the strobe doesn't have a ready light (which would be weird), wait a long time between shots -- 30 seconds or more, just to be sure -- before firing. If you can't make the problem happen in this experiment, then you've learned that the strobe has a long recycle time.

FWIW, Ikelite lists the SS-400's recycle time as 3.5 seconds. I would expect the 300's to be longer, as it is an older unit.

Old nicads wouldn't cause this probleb. Nicads have this wonderful property of delivering full current right up to the point where they're almost fully exhausted. An old nicad pack would simply result in fewer total flashes from the system...BTW, are you sure the thing has nicads in it? Old alkalines *would* slow down the recycle time.
