The setup Dave described is best, but I haven't ever done it.

The hard way is that I carry a dive light in my left hand. You can actually get pretty good at it after a while, but it's a pain. The modeling lights inside my strobe are not great dive lights. Search a reef with your camera isn't very ergonomic, so I carry a dive light.

The easy way is a dive buddy who's been diving with you for many years. She instinctively uses her light to get me my focus. She finds things and reads my mind underwater.
